In Ayurveda, gastritis is categorized primarily as Amlapitta, a condition where the sharp, sour, and acidic qualities of Pitta dosha are severely aggravated. This causes inflammation, erosion, and burning sensations in the stomach lining.

Key Ayurvedic Herbs for Gastritis

Ayurvedic herbal strategies focus on cooling the heat (Sheeta), neutralizing stomach acid, and protecting the mucosal barrier.

  • Yashtimadhu (Licorice / Glycyrrhiza glabra): It acts as a natural mucosal protective agent, helping to regenerate the gastric lining and calm inflammation.
  • Amalaki (Indian Gooseberry / Phyllanthus emblica): Rich in antioxidants, it directly lowers oxidative stress in the gut and gently reduces excess hyperacidity without causing rebound effects.
  • Shatavari (Asparagus racemosus): Known for its cooling (Pitta-shaming) properties, it coats the stomach lining and acts as a soothing demulcent.
  • Guduchi (Tinospora cordifolia): Acts as an immunomodulatory adaptogen, which is particularly beneficial for cases with an autoimmune component like atrophic gastritis.
  • Avipattikara Churna: A classic, multi-herb composite powder widely used to clear heat, promote proper gut motility, and eliminate sour belching.

Panchakarma for Gastritis

When conservative herbal management (Shamana) is insufficient to balance the doshas, purification therapies (Shodhana) via Panchakarma are utilized to expel excess morbid Pitta from the upper and lower gastrointestinal tract.

Panchakarma ProcedureTherapeutic IntentIndications (When to use)Contraindications (When to avoid)
Virechana
(Medicated Purgation)
The primary choice for Amlapitta. It moves metabolic waste downward to radically clear excess Pitta from the liver, spleen, and small intestine.• Chronic hyperacidity
• Persistent bile reflux
• Skin flare-ups linked to gut heat
• Stable, non-bleeding chronic gastritis
• Severe acute dehydration
• Emaciation or physical weakness
• Active, bleeding gastric ulcers
• Pregnancy
Vamana
(Therapeutic Emesis)
Induced vomiting to clear toxins from the stomach and upper chest cavity, targeted when Kapha mixes with Pitta (Urdhwaga Amlapitta).• Severe morning nausea
• Thick, mucus-heavy sour belching
• Heavy feeling in the abdomen
Strictly avoided in acute or erosive gastritis
• Esophageal varices
• High risk of gastrointestinal bleeding
• Heart disease or uncontrolled hypertension

Clinical Evidence and Scientific Studies

Modern medical research validates several aspects of traditional Ayurvedic gastroenterology:

  • Mucosal Healing and Acid Management: A formal pilot clinical study on Amlapitta Mishran Suspension (a standard liquid formulation containing Yashtimadhu, Guduchi, and Neem) demonstrated that it significantly neutralizes stomach acid, reduces harmful pepsin activity, and actively accelerates the healing of gastric erosions and ulcers.
  • Symptom Reduction: Published clinical case data tracking the use of classic formulas like Avipattikara churna and Shatavari confirm a statistically significant reduction in the core symptoms of gastritis, including chest burning (Urodaha), indigestion (Avipaka), and nausea (Utklesha) over a 4 to 8-week treatment block.
  • Mechanisms of Action: In vitro and in vivo studies highlight that these herbs work through direct anti-inflammatory pathways (inhibiting pro-inflammatory cytokines), delivering rich antioxidant properties, and safely modulating the gut microbiota.
  • Research Limitations: While results are highly promising for complementary care, systematic reviews point out that many current studies rely on small sample sizes or animal models. There is a continuous push for larger, double-blind randomized controlled trials (RCTs) to fully map long-term safety and evaluate potential herb-drug interactions with standard medications like PPIs
